Job summary

MJHS seeks a Social Work Care Manager to support Elderplan and HomeFirst members by addressing medical, psychosocial, physical and spiritual needs over the phone. You will develop and monitor person-centered care plans, coordinating with the interdisciplinary team, PCPs, and member representatives to ensure continuity of care.

Qualifications

• Bachelor's degree in social work or related field required.
• MSW or LMSW preferred.
• Minimum three to five years clinical social experience in long term care, managed care, home care, geriatrics or hospice preferred.
• Prior experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations preferred.
• Prior experience in an interdisciplinary service delivery environment preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic support to members’ medical, psychosocial, physical and spiritual needs.
• Develop, implement, and monitor person-centered service plans.
• Ensure continuity of care across all healthcare settings by collaborating with the interdisciplinary team, PCP, member or member’s representatives, and specialists.
• Educate members on disease processes, preventative health, and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
