Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team for telephonic patient consultations on specialty medications. You will perform medication reconciliation, screen for drug interactions, and provide side effect management, all while meeting production metrics and quality standards.

The role emphasizes patient-centered care, collaboration with physicians, and ongoing learning in specialty therapies. A licensed Pharmacist with strong clinical background is required.

Qualifications

• ACPE-accredited School of Pharmacy degree (PharmD or B.S. Pharmacy).
• Current, unrestricted pharmacist license in state of residence; must maintain licensure with education credits.
• At least two years of licensed pharmacist experience preferred.
• Strong clinical background.
• Experience with Specialty Pharmacy medications desirable.
• Participate in compliance program objectives.
• Ability to interact cooperatively with others.

Responsibilities

• Provide telephonic patient consultations for specialty medications.
• Document consultations in pharmacy software per internal policies and URAC compliance.
• Interpret and evaluate prescriptions; contact providers/patients to verify details and expedite orders.
• Contact physicians for new prescriptions; discuss transfers; promote generics when possible.
• Verify prescription information entered in the system; answer clinical questions.
• Stay current with specialty therapies and disease state guidelines.
• Back up other pharmacists; train less experienced staff.
• Document and report medication errors; support quality initiatives.
• Comply with federal/state laws and highest ethical standards.
• Other duties as assigned.
