Job summary

Lumicera seeks a Pharmacist, Clinical Consultation to perform telephonic patient consultations regarding specialty medications and disease-state management. The role includes medication reconciliation, drug interaction screening, and side-effect management, with independent work and high patient-centered care standards.

The successful candidate will meet licensure requirements, have strong clinical skills, and contribute to quality metrics while supporting physicians and pharmacists across care

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy; BS or PharmD required.
• Current, unrestricted pharmacist license in state of residence and active licensure.
• At least two years of licensed pharmacist experience preferred.
• Strong clinical background; familiarity with specialty pharmacy desirable.
• Commitment to compliance objectives and collaborative teamwork.

Responsibilities

• Conduct telephonic patient consultations for specialty medications, focusing on disease state management.
• Document clinical interventions in pharmacy software; ensure regulatory compliance.
• Review prescriptions for strength, directions, and diagnosis; expedite processing.
• Contact providers for prescription clarification and coordinate transfers as needed.
• Support QA standards, resolve patient issues, and adapt to changing workloads.
