Job summary

Qualifications

• Graduate from an accredited Nursing Program.
• Massachusetts RN license required.

Responsibilities

• Coordinate and facilitate quality clinical management for outpatient population across continuum of care.
• Collaborate with providers to identify needs for preventive care and chronic disease management.
• Provide skilled telephonic triage and assessment.
• Perform direct patient care including wound care and point of care testing.
• Educate and train Medical Assistants and patients on health issues and care plans.
• Manage vaccination administration and medication per orders and policy guidelines.
• Ensure accurate documentation in the Electronic Health Record and coordinate referrals.
