Job summary

3rd Street Youth Center & Clinic in San Francisco seeks a TAY Residential Case Manager to provide low-barrier, harm-reduction case management for youth (ages 18–27) at the Navigation Center. You will handle intake, service planning, and ongoing engagement, helping guests obtain housing, benefits, and health services while coordinating with coordinators and partners.

The role requires trauma-informed practice, group facilitation, and strong documentation skills; CIH/HSH policies guide daily

Qualifications

• Bachelor's degree or equivalent work experience.
• 2–3 years of experience working with high-risk and homeless youth.
• Experience facilitating groups preferred.

Responsibilities

• Conduct intake and program orientation for guests within 24 hours of arrival.
• Create housing-focused service plans with goals and identified barriers.
• Plan, facilitate, and run support groups and community activities for youth.
• Assist guests with Medi-Cal, CalFresh, CAAP and other benefits.
• Maintain confidential guest files and accurate documentation.
