Job summary

3rd Street Youth Center & Clinic in San Francisco’s Bayview/Hunters Point neighborhood seeks a YAP Case Manager/Problem Solver to help youth access housing solutions and navigate the coordinated entry system. You will assess needs, coordinate with the housing network, and advocate for timely supports.

You will coordinate casework, maintain records, attend housing meetings, and partner with staff to provide comprehensive wrap-around services while upholding strict confidentiality and professional

Qualifications

• Bachelor's degree or equivalent work experience.

Responsibilities

• Youth Coordinated Entry intake.
• Provide problem solving assistance for immediate housing solutions.
• Case coordination and advocacy with the agency and partner organizations.
• Documentation and recordkeeping in the assigned database.
• Meet with rehoused clients twice a month or as needed.
• Develop and implement policies for the Youth Access Point.
• Design and maintain the YCE access office.
• Provide immediate support to youth (hygiene supplies, food, etc).
• Develop individual support and transition plans with youth.
• Attend community, city, and regional housing meetings.
• Support staff and Youth in 3rd Street programming as needed.
• Maintain confidentiality and professional standards.
