Job summary

Honor Community Health is seeking a Full Time Exempt, Level 1 Targeted Case Manager to support individuals in sustaining mental health and substance use recovery. The role reports to the Director of Care Management, with standard hours Monday–Friday 8:30am–5:00pm.

Qualifications include a bachelor’s degree in social work or a related field with licensure as an RN, plus at least one year of direct service experience in behavioral health. Benefits include a comprehensive package and CME allowance.

Qualifications

• Experience providing direct services to clients with mental health and/or substance use disorder symptoms.

Responsibilities

• Provide targeted case management services within a Certified Community Behavioral Health Clinic (CCBHC).
• Assist individuals in accessing needed medical, behavioral health, social, educational, legal, and community services.
• Help clients resolve barriers to independence and sustain themselves in the community.
