Job summary

Mental Health Association is seeking a full-time Case Manager to join our Integrated Case Management Services Team. You will establish therapeutic relationships and provide intensive community-based services to up to 20 consumers, coordinating care and advocating for access to services.

The position offers an annual base range of $45,650 to $47,300, plus fully paid medical coverage, a generous 401(k) match, and plentiful paid holidays, vacation, personal and sick time.

Qualifications

• Bachelor’s degree in a behavioral health field from an accredited institution and one year of relevant, post-degree experience; or an Associate’s degree in a direct care field and two years of relevant post-degree experience; or four years of relevant work experience with mental health consumers.
• Able to work a flexible schedule, including evenings, weekends, and holidays.
• Valid Driver’s License.

Responsibilities

• Establish and maintain ongoing therapeutic relationships and case manage up to 20 mixed risk level consumers.
• Provide intensive community-based services to maximize access to services and community functioning.
• Develop and implement service plans and complete required documentation.
• Facilitate access to services, including transportation and entitlements.
• Coordinate and integrate services from multiple providers until discharge from ICMS.
• Monitor service delivery and advocate for consumer needs.
• Identify consumer resource gaps and support the consumer’s natural support system.
