Job summary

BrightView Health is seeking a compassionate, entry-level Targeted Case Manager I to support patients with Substance Use Disorder. You will assess needs, connect patients with medical, social, education, and employment resources, and document case management activities in the electronic health record.

Requirements include a Bachelor's degree in a behavioral science field and at least 1 year of experience in human services.

Qualifications

• Excellent communication and empathy for patients in recovery.
• Ability to work with a diverse patient population.
• Strong time management and problem-solving skills.

Responsibilities

• Assess patient eligibility and develop individualized action plans.
• Refer patients to resources and monitor adherence with follow-ups.
• Document needs assessment and case management activities in the EHR.
• Provide crisis support and ensure transportation to care providers.
• Attend supervision and treatment team meetings and maintain compliance.
