Family Health Center – Southwest Indiana is seeking a Case Manager to provide coordinated, client-centered services for individuals with mental health and co-occurring disorders. The role focuses on assessments, planning, coordination, and connection to community resources.
The position is 1.0 FTE, 40 hours per week. Essential duties include comprehensive assessments, developing PCTPs, providing services, and advocating for access to housing, benefits, and healthcare.
The Case Manager provides coordinated, client-centered services to individuals and or their families experiencing both mental health challenges and substance misuse. This role supports clients in achieving stability, independence, and improved quality of life by assessing needs, developing service plans, coordinating care, and connecting clients and or their families to community resources.
This is a 1.0FTE position, working 40 hours a week. Hours TBD
The essential functions may include, but are not limited to the following:
Education: Bachelor’s degree in a field related to mental health
Certification: N/A
Licensure: Valid Driver’s license; must be insurable under the organization’s vehicle insurance policy.
