Job summary

Westmoreland Casemanagement And Supports Inc. is seeking a full-time Targeted Case Manager to assess client needs and develop comprehensive service plans for both children and adults.

You will conduct assessments, link clients to housing and vocational resources, and coordinate care with an interdisciplinary team. The role requires flexibility for after-hours on-call rotation, accurate documentation, crisis intervention, and active community engagement.

Qualifications

• Bachelor’s degree in social services or related field; casemanagement experience preferred.
• Two years direct contact with Behavioral Health clients; two years as a TCM Level 1 at WCSI (or alternate path).
• Strong interpersonal and communication skills; valid PA driver’s license and auto insurance; required screenings.

Responsibilities

• Assess client needs using Environmental Matrix and SBA; develop and implement a comprehensive Targeted Case Management Service Plan.
• Respond to urgent needs with crisis intervention; participate in 24-hour on-call rotation.
• Monitor all services, coordinate resources, and maintain current directory of community resources; advocate for client.
• Maintain Targeted Case Management records, ensure regulatory compliance, and prepare for audits.
• Engage with community partners; participate in outreach events and represent the organization.
• Prepare reports, communicate effectively, and collaborate with interdisciplinary teams.
