Job summary

Advanced Behavioral Health, Inc. seeks a Targeted Case Manager to support youth with behavioral challenges or suicidal ideation. You will assess needs, develop comprehensive care plans, and connect families with appropriate resources to promote well-being and success.

This family-centered program emphasizes wraparound treatment, partnerships with local agencies, and broad community-based support. The role may involve travel within the local area and coordination with crisis services.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, or related discipline.
• Experience with youth populations is preferred.
• Knowledge of community resources and social services.

Responsibilities

• Complete comprehensive needs assessments for children, adolescents, and families in crisis.
• Establish treatment programs, coordinate services, arrange resources, and provide transportation as needed.
• Identify and access community resources to support clients’ goals.
• Develop partnerships with local organizations to expand resources for youth.
• Advocate for clients to ensure access to services.
• Provide services in homes, schools, or community settings.
• Coordinate with Crisis Intervention for emergencies and risk factors.
• Develop safety plans and connect clients with immediate assistance.
• Facilitate workshops and individual sessions on budgeting, job readiness, communication, and decision-making.
• Track progress and outcomes as per care plans.
• Evaluate program effectiveness through data and feedback.
• Ensure compliance with CARF/COMAR/HIPAA and EMR standards.
