Job summary

Westmoreland Casemanagement and Supports is seeking a full-time Targeted Case Manager to assess client needs and develop comprehensive service plans for children, transitional age, and adults. The role focuses on housing, vocational/educational goals, and social determinants of health, requiring knowledge of child, transitional, and adult systems.

You'll monitor services, coordinate resources, advocate for clients, and maintain records.

Qualifications

• Bachelor’s degree in social services related field.
• Associate degree in Human Service/Social Science with two years direct client contact and two years as a TCM Level 1.
• High school diploma with 12 semester credit hours in sociology, social welfare, psychology, gerontology, or related field and two years of relevant experience.

Responsibilities

• Assess client needs using Environmental Matrix and SBA; develop, prepare, and implement a comprehensive Targeted Case Management Service Plan.
• Monitor all services received by clients and coordinate resources; maintain records and complete paperwork.
• Provide crisis intervention and participate in rotating after-hours on-call system for urgent needs.
