Job summary

Westmoreland Casemanagement and Supports is seeking a full-time Targeted Case Manager to assess client needs, develop and implement service plans, and coordinate resources for both child and adult populations. The role requires flexibility to work evenings and weekends for after-hours on-call rotation and outreach events.

Primary duties include monitoring services, maintaining records, and advocating for clients within a multidisciplinary team.

Qualifications

• Bachelor’s degree in a social services related field is required.
• Associates degree plus two years’ direct mental health experience is an alternative.
• High school diploma with related coursework and two years of TCM experience is accepted.

Responsibilities

• Assess client needs using Environmental Matrix and SBA; develop and implement a comprehensive Targeted Case Management Service Plan.
• Provide crisis intervention and ensure alternatives to hospitalization are explored.
• Monitor services, coordinate resources, and advocate for clients across systems.
