Responsibilities

• Provides basic patient care including comfort, vital sign measurement, hygiene, and mobility assistance.
• Acts as liaison between patient and nurse to report changes and concerns.
• Delivers high-level customer service to patients, families, visitors, and staff.
• Documents patient care accurately and promptly.
• Participates in hand-off/report of pertinent information on arrival and departure.
• Performs delegated tasks once competency is validated.
• Uses therapeutic transfer techniques to mobilize patients safely.
• Encourages patient independence and personal task completion.
• Individualizes care based on patient interests and hobbies.
