Qualifications

• Proven ability to provide basic patient care and comfort.

Responsibilities

• Provide basic patient care including daily living activities and vital signs.
• Report changes to nurses and care team promptly.
• Deliver high-quality customer service to patients and families.
• Document relevant patient information accurately and timely.
• Participate in hand-off reports and transfer of information.
• Perform delegated tasks once competency is validated.
• Assist with patient mobility using safe transfer techniques.
• Encourage patient independence and personalize care based on interests.
