Methodist Le Bonheur Healthcare in Memphis is seeking a Registered Nurse to serve as a consistent care coordinator across the continuum of care. You will assess patients’ physical, psychological, and social needs and coordinate communication with providers to improve the patient and family care experience.

The role emphasizes education, screening, prevention, and follow-up care within a clinical/office setting and aligns with MLH Mission, Vision and Values.

Qualifications

• Associates Degree in Nursing required.
• Bachelor's degree in Nursing preferred.
• Active Registered Nurse license in Tennessee required.

Responsabilités

• Assess the physical, psychological, and social needs of the patient and family.
• Create a seamless flow through the health system to facilitate care.
• Coordinate education, resources and activities for patients and families.
• Maintain and document patient data accurately.
• Perform clinical nursing procedures within scope and training.
• May perform telephone duties within RN role.
• Perform other duties as assigned or requested.
