Methodist Le Bonheur is seeking a Registered Nurse to serve as a consistent care coordinator across the continuum of care for patients within our health system. You will assess the needs of patients and families, coordinate communication with providers, and deliver education, screenings, and follow-up care.

Responsibilities include maintaining patient data, performing clinical procedures, and potential RN telephone duties.

Qualifikationen

• Associates Degree in Nursing is required.
• Bachelor's Degree in Nursing is preferred.

• Assesses the physical, psychological, and social needs of the patient and improves the care experience.
• Creates a seamless flow through the health system to facilitate the patient’s care.
• Develops and coordinates education, resources, and activities for patients and families.
• Maintains reliable systems to document, track and monitor patient data.
• Performs clinical procedures within the scope of nursing practice and clinical training.
• May perform telephone duties within the RN role.
• Performs other job functions as assigned.
