Job summary

Methodist Le Bonheur is hiring a Registered Nurse to serve as a consistent care coordinator across the continuum of care. You will assess patients’ physical, psychological, and social needs and improve their care experience.

You will coordinate communication with providers and support education, screening, prevention, and follow-up care. The role requires nursing licensure in Tennessee or eligibility, an Associates degree in nursing, and preferably a Bachelors in Nursing.

Qualifications

• Associates Degree in Nursing is required.
• BLS/CPR certifications are required.
• Tennessee RN license or eligible licensure is required.

Responsibilities

• Assesss the physical, psychological, and social needs of patients and improve care experience.
• Coordinate communication with providers on behalf of the patient.
