Qualifications

Responsibilities

• Provides direct nursing care in accordance with policies, procedures, and protocols.
• Implements and monitors patient care plans.
• Monitors, records, and communicates patient condition.
• Serves as a primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient discharge planning needs with patient/family and healthcare team.
• Assesses learning needs of patient and family and provides education.
• Performs job duties in accordance with the Nurse Practice Act.
