Job summary

Easterseals MORC is seeking a Supports Coordinator to serve Grand Rapids, MI. This on-site role focuses on case management, connecting individuals with services, and developing person-centered plans for recovery and independence.

The successful candidate will hold a Bachelor's degree in relevant fields, have at least one year of experience with mental disability services, and possess a Michigan driver's license. PSLF benefits and other support options are highlighted in the perks.

Qualifications

  • Bachelor's Degree from an accredited college or university.
  • Experience: at least 1 year working with a person with a mental disability.
  • Valid Michigan Driver's License.

Responsibilities

  • 1. Engage individuals in a welcoming, hopeful, empathic manner regardless of disability or recovery phase.
  • 2. Screen for co-occurring disorders and identify substance abuse criteria.
  • 3. Assess needs, maintain caseload, develop goals with stage-based plans; complete referrals and docs.
  • 4. Advocate and connect to supports, utilizing community resources.
  • 5. Educate on social determinants of health and resources.
  • 6. Identify relapse triggers; develop relapse prevention and crisis plans.
  • 7. Maintain productivity with an average of three billable hours per week per client.
  • 8. Provide supports coordination for individuals, families, and guardians; monitor living arrangements and placements.
  • 9. Attend team meetings to review problems and plan solutions.

Qualifications:

  • Bachelors Degree from an accredited college or university and one year of experience working with a person with a mental disability
  • Valid Michigan Drivers License

Duties and Responsbilities:

1. Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery.

2. Screen for co-occurring disorders and recognize diagnostic criteria used to identify substance abuse or dependency.

3. Assess and evaluate the needs of individuals and continues caseload contact to develop goals. Demonstrate the ability to identify stage of change and use interventions consistent with stage of treatment. Assist clients in developing strength-based, stage wise treatment matched person-centered plans that are designed to address identified needs. Complete referral forms, clinical and legal documentation necessary to obtain community-based services for clients as well as collaboration and communication with other SA/MH community partners.

4. Advocate and provide options for supports and services as needed and seeks out and develops community resources to meet the needs of assigned individuals for activities and support services.

5. Educate individuals and provide resources for social determinant of health needs.

6. Assist individuals with identifying changes in behavior, attitude, feelings and thoughts that are triggers for relapse. Assist in the development of a relapse prevention plan and crisis plan utilizing a variety of supports including personal, community and treatment interventions.

7. Provides services and supports to ensure that productivity standards are maintained an average of three billable hours (inclusive of direct and indirect services) per week per client.

8. Provides supports coordination services for individuals, families and/or guardians for support, rehabilitation and/or crisis intervention purposes. Monitors and evaluates living situations, school and/or program placements of individuals to determine consistency of treatment and progress. Assists in commitment procedures when necessary as well as voluntary admissions. Provides education on self-help options such as 12 Step programs, Dual Recovery, and other peer support groups.

9. Attend team meetings to review problems and plan solutions.
