Responsibilities

• Monitor treatment and support plans; notify the appropriate I/DD Care Coordinator when needed.
• Gather and compile personal and demographic information.
• Understand rules governing care and support for individuals with developmental disabilities.
• Attend planning meetings using person-centered principles and coordinate supports and community services.
• Coordinate transitions into personal residences and broker services/supports.
• Link with Medicaid and other health care providers for supports.
