Job summary

WEST MICHIGAN COMMUNITY MENTAL HEALTH SYSTEM is seeking a Supports Coordinator to provide comprehensive coordination services for individuals with mental illness, co-occurring conditions, and developmental needs. The role includes linking clients to CMH and community agencies, monitoring progress, and advocating for consumer needs.

The position supports crisis stabilization efforts and may require on-call hours, including nights, weekends, and holidays.

Qualifications

  • Bachelor's degree in a human services field or equivalent professional education.
  • Must meet Medicaid provider qualifications and QMHP/QIDP/CMHP eligibility.
  • SUD certification (CADC or equivalent) or willingness to obtain.
  • Valid driver's license and own transportation to multiple locations.

Responsibilities

  • Provide supports coordination services for individuals with mental illness and co-occurring disorders.
  • Coordinate with CMH and community agencies to plan and link services.
  • Monitor service effectiveness and consumer satisfaction; advocate for consumers.
  • Collaborate with Medicaid Health Plan and other providers as needed.
  • May include on-call hours, nights, weekends, and holidays as part of Crisis Stabilization efforts.

The Supports Coordinator is responsible for providing supports coordination services to individuals with mental illness, co-occurring substance use disorders, children with serious emotional disturbance and/or all persons with intellectual/developmental disabilities under the supervision of the population team leader or coordinator. These services may be one or more of the following: collaborating on service planning utilizing person centered planning principles, linking and coordinating to CMH and community services and community agencies, monitoring for service effectiveness, consumer satisfaction and serving as a consumer advocate and coordination with the Medicaid Health Plan or other health care providers. Consumers may require long-term support and intervention to maintain or improve functioning and have compromising physical or interfering behavioral conditions. This position is part of a staff pool that serves to fulfill the agency’s Crisis Stabilization Service. This may require on-call hours including nights, weekends, and holidays.

Position Requirements
  • A minimum of a Bachelor’s degree in a human services field, such as education, social work, physical therapy, occupational therapy, speech language pathology, audiologist, registered nurse, registered dietician, therapeutic recreation, art/dance therapy, behavior analyst, child development, counseling/guidance, criminal justice, health administration, health education, music therapy, psychology, sociology, child welfare and protection, juvenile justice, gerontology, or rehabilitation counseling.
  • OR
  • State of Michigan licensure, certification, or registration as required by one of the following professions: psychologist (LP, LLP, TLLP), physician (MD or DO), educator from an accredited program*, social worker (LMSW, LLMSW, LBSW, LLBSW), physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed professional counselor, licensed marriage and family therapist, or a licensed physician’s assistant. If not currently licensed, certified, or registered in one of these professions, the individual must be eligible and willing to apply for and obtain such licensure, certification, or registration immediately per the guidelines for the profession. (*Educators are not required to retain their teaching certification in order to be considered qualified for their position.)
  • Must meet the Medicaid provider qualifications for services for the specific program; be a Qualified Mental Health Professional (QMHP), Qualified Intellectual Disability Professional (QIDP), and/or a Children’s Mental Health Professional (CMHP) or must be eligible to obtain the designation.
  • In addition, to provide SUD specific services, the individual must have or be eligible to obtain certification for one of the following: Certified Alcohol and Drug Counselor, Certified Advanced Alcohol and Drug Counselor, or a Certified Co-Occurring Disorders Professional – Diplomate through the Michigan Certification Board for Addiction Professionals (MCBAP) or enter into a developmental plan to obtain such certification following the requirements of MCBAP.
  • Must be credentialed and privileged to practice at WMCMH by the Executive Committee of the Clinical Oversight Committee of WMCMH.
  • Must possess a valid driver’s license and provide own transportation to and from meetings and activities at varying work locations including all agency locations in Lake, Mason, and Oceana Counties.
  • Must be certified in First Aid and CPR or obtain training after hire
  • Lived experiences with mental illness/developmental disabilities/substance use disorders are valued.

Full-Time/Part-Time Full-Time

Pay Range $24.52 - $30.98
