Job summary

Zachary Piper Solutions is seeking a Supports Coordinator to join a healthcare and social assistance organization serving individuals across Chester, Montgomery, and Delaware Counties in PA. The role focuses on coordinating services, monitoring care, and connecting clients with resources to live safely and independently.

You will oversee a caseload of 40–50 individuals, conduct in-person visits, coordinate transportation and services, maintain records, and communicate with families and providers

Qualifications

• Experience in human services, case management, or related field preferred.
• Ability to manage a high-volume caseload with organization.
• Strong communication with clients, families, and providers.
• Experience documenting information and deadlines in records.

Responsibilities

• Oversee a caseload of 40–50 individuals and be a consistent point of contact.
• Complete scheduled in-person visits with individuals and families.
• Coordinate transportation, employment services, medical care, and other supports.
• Review needs to align with support plans and goals.
• Maintain accurate case records and required paperwork.
• Communicate with clients, families, providers, and community partners.
• Track services and perform ongoing follow-up to ensure support.
