• Experience in human services or case management preferred.
• Strong organizational and time-management skills.
• Excellent communication and relationship-building.
• Proficient with Microsoft Office, especially Excel.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Manage a caseload of 40–50 individuals with advocacy for their needs.
• Conduct in-person visits with clients, families, caregivers, and providers.
• Coordinate supports including transportation, employment services, medical care, housing, and community resources.
• Review needs to ensure services align with support plans and goals.
• Maintain accurate case records, monitoring documentation and reports.
• Communicate regularly with clients, families, providers, and community organizations.
• Track services and follow up to ensure ongoing support.
