Responsibilities

• Manage an assigned caseload of individuals with intellectual and developmental disabilities.

• Coordinate transportation, employment assistance, medical appointments, and community resources.
• Gather, document, and maintain detailed case information and monitoring reports.
• Provide ongoing follow-up to ensure services remain in place and needs are addressed.
