Qualifications

• Bachelor's degree in Psychology, Social Work, or related field preferred.
• Strong organizational and time-management skills.
• Valid driver's license and ability to travel locally.

Responsibilities

• Oversee a caseload of approximately 40–50 individuals, serving as a consistent point of contact and advocate for their needs.
• Complete scheduled in-person visits with individuals, families, caregivers, and service providers to evaluate services, safety, and overall well-being.
• Help coordinate a variety of supports, including transportation, employment services, job coaching, medical care, residential programs, and community-based resources.
• Review individual needs and help ensure services align with established support plans and goals.
• Maintain accurate case records, monitoring documentation, reports, and other required paperwork.
• Communicate regularly with clients, families, providers, and community organizations to address concerns and follow through on outstanding needs.
• Track services and complete ongoing follow-up to ensure individuals continue receiving appropriate support.
