Job summary

Zachary Piper Solutions seeks a Supports Coordinator to manage a caseload of individuals with intellectual and developmental disabilities in the Chester, PA area. You will conduct in-person visits, assess needs, coordinate transportation, employment and medical services, and document case information to ensure health, safety, and quality of life.

Qualifications

• Minimum experience in human services or case management preferred but not required.
• Proficiency with Microsoft Office applications, including Excel.
• Ability to manage a high-volume caseload with organization and follow-up.

Responsibilities

• Manage an assigned caseload of approximately 40-50 individuals with IDD, ensuring needs are met.
• Conduct regular in-person monitoring visits with individuals, families, and providers.
• Coordinate and facilitate support services including transportation, employment assistance, and medical appointments.
• Gather and maintain detailed case information and documentation.
• Follow up to ensure services remain in place and needs are addressed.
