Job summary

CAMBA in Brooklyn, NY is seeking a dedicated Case Manager to provide intake, assessments, service planning, referrals, advocacy and follow-up for clients in the Stability Supportive Housing program.

Full-time, 35 hours per week, you will document progress, coordinate with healthcare, housing and legal services, and ensure compliance with funding requirements while maintaining audit-ready charts.

Qualifications

• Bachelor’s degree with one year of experience, or an Associate degree with two years, or a high school diploma with four years.
• Experience in homeless, mental health, or substance use service settings preferred.
• Excellent written and verbal communication skills.
• Computer literacy in Microsoft Office Suite.

Responsibilities

• Conduct intakes, assessments, service plans and reassessments.
• Enter client services into Client Track and maintain accurate records.
• Connect clients to medical, mental health, substance use, legal, and housing services.
• Monitor progress and document outcomes with regular contact and visits.
• Advise clients and coordinate with outside organizations; escort to appointments.
• Prepare monthly program reports and maintain audit-ready charts.
