Job summary

CAMBA is seeking a Case Manager to support the Scattered Site Housing Program in Brooklyn. You will ensure clients access needed services and maintain stable housing, with attention to documentation and program compliance.

The role requires a Bachelor's (or equivalent) with at least 4 years in case management, strong communication, and Microsoft Office skills. This is a full-time position at 35 hours per week, based at 19 Winthrop Street, Brooklyn NY 11225.

Qualifications

• Good written & verbal communication.
• Computer literacy in Microsoft Office Suite.

Responsibilities

• Provide contract funded activities: intakes, assessments, service plans, reassessments, referrals, advocacy, housing inspections.
• Connect clients to medical, mental health, substance use treatment, as well as legal services.
• Monitor clients' progress via regular calls or home/office visits and document progress notes.
• Function as liaison with outside organizations regarding education, healthcare, housing and legal issues.
