Pathfinder Services, Inc. in Plymouth, IN is seeking a Supported Living Assistant Manager to supervise DSPs, coordinate care, and support daily operations in safe, person-centered homes where individuals can thrive and live independently.

The role entails leading staff meetings, managing scheduling, and ensuring accurate documentation and Medicaid-related processes, while collaborating with families and leadership to maintain high-quality supports.

Qualifications

• Minimum high school diploma or GED.
• Prefer 2 years experience in human services.
• Must be at least 18 years old.
• Basic language, written and computer skills for documentation.
• Demonstrated ability to support people with disabilities.
• Ability to work a flexible schedule if needed.
• Crisis prevention, intervention and teaching skills.
• Valid driver’s license and insured vehicle.
• Ability to read, write and speak English.

Responsibilities

• Oversee DSPs across residential sites and coordinate staffing.
• Lead team meetings and monitor training compliance.
• Support PCISPs, appointments, and transportation needs.
• Ensure homes meet safety, health and quality standards.
• Maintain case notes, billing records, and Medicaid documentation.
• Communicate with families and leadership; ensure shift coverage.
