Job summary

Pathfinder Services, Inc. is seeking a Supported Living Manager-2 in Wabash, IN to lead two homes and supervise Direct Support Professionals while delivering person-centered services.

The role focuses on advocacy, coordination of daily supports, and ensuring compliance with Medicaid and internal policies. You will mentor staff, foster meaningful community connections, and support individuals toward greater independence through ISP-guided activities.

Qualifications

• Minimum of a high school diploma/GED.
• Prefer 2 years experience in the field of Human Services.
• At least 18 years of age.
• Basic language, written, and computer skills necessary for communication and documentation.
• Demonstrated ability to support people with disabilities.
• Ability to work a flexible schedule if needed.
• Crisis prevention, intervention, and teaching skills.
• Valid driver's license and current auto insurance required
• Ability to speak, hear, and read the English language.

Responsibilities

• Lead with person-centered support and supervise DSPs.
• Coordinate staffing, transportation, and daily activities.
• Ensure ISP goals and safety measures are implemented.
• Maintain accurate documentation and Medicaid-related records.
• Collaborate with families, guardians, and the interdisciplinary team.
