Job summary

Support Management, doing business as Community Intersections, seeks a Day Support Specialist to assist adults with IDD in-home, base site, or community settings. You will provide training and supervision in areas like community access, supervision, household tasks, employment, personal care, and money management.

The role involves coaching, encouraging, and documenting progress, with opportunities to help individuals live more independently within their local community.

Qualifications

• Good written and verbal communication skills.

Responsibilities

• Work with assigned individuals according to weekly schedule.
• Teach/role model the skills identified by the IP (Individidualized Plan).
• Support the person to develop connections in their local community.
• Assist people served with medical/personal care needs including, but not limited to, changing/restroom assistance.
