Job summary

Delta Community Supports, Inc. seeks a caring Community Center Professional to promote community support for individuals with developmental and intellectual disabilities in a program setting. You will provide direct care, training, and outcome-based supports under supervision.

Responsibilities include implementing ISP/ICP actions, maintaining training, and documenting progress while ensuring health and safety. A valid driver’s license and prior relevant experience are required.

Qualifications

• High school diploma or GED required.
• At least two years’ experience in the field of intellectual disabilities.
• Valid driver’s license in the state of residence.

Responsibilities

• Engage individuals according to our mission.
• Utilize programmatic resources.
• Monitor the implementation of high quality individual support plans.
• Promote and follow Delta’s Risk Management Program for health and safety.
• Maintain all required training per Delta policy and state regulations.
• Provide for the care, safety and well-being of individuals who attend the Community Center, participate in Community Participation Supports in the community, and receive In Home Supports.
• Work cooperatively with other Community Support Professionals in the implementation of various activities for groups of individuals.
• Conduct and write annual interest/skill assessments for assigned individuals.
• Participate as a team member in the ISP/ICP process for assigned individuals.
• Implement ISP/ICP outcomes and other specified supports for designated individuals.
• Complete routinely required paperwork in a timely and accurate manner.
• Actively participate in routinely scheduled meetings and trainings.
• Communicate continuously with your Supervisor.
