Job summary

Imagine the Possibilities, Inc. is seeking Direct Support Professionals to empower individuals to achieve their personal goals across daily life, community participation, and independent living.

You will collaborate with care teams to develop goals, implement behavioral plans, document progress in Setworks, and provide safe, compassionate support in home and community settings, including transportation and daily living activities.

Qualifications

• Excellent communication and listening skills.
• Ability to comply with care plans and documentation.
• Ability to work in a 24/7 service environment.

Responsibilities

• Develop individualized goals and provide direct support focused on teaching skills and independence.
• Apply approved behavioral strategies while upholding rights per plans.
• Document service details and responses accurately in Setworks.
• Provide hands‑on support in home and community settings including daily living and transportation.
• Administer and document medications per six rights and maintain reliable attendance.
