Job summary

The role involves traveling to members’ homes, coordinating services with a multidisciplinary team, and assessing needs across medical, behavioral health, and social domains. Qualified candidates include RNs licensed in Tennessee or compact states or MSWs with active unrestricted licenses.

Qualifications

• Clinical experience required (2 years).
• Strong time management, written and verbal communication.
• Interpersonal and organizational skills.

Responsibilities

• Partner with members and families to identify needed supports and direct services.
• Collaborate with clinical and social support colleagues to meet health and living needs.
• Conduct thorough face-to-face visits and assess member status across domains.
• Apply criteria to authorize home and community-based services and ensure delivery.
• Maintain a valid driver's license and comply with safety policies.
• Complete required immunizations and digital literacy assessment as applicable.
