Job summary

WTC Support Services, Inland Empire, CA, seeks a part-time Support Coordinator to manage a caseload under Coordinated Family Support (CFS). You will conduct assessments, develop individualized service plans, and coordinate resources with Regional Centers, healthcare providers, schools, and community agencies.

You will advocate for families, ensure timely documentation, and collaborate with partners to help participants reach goals within a flexible hybrid schedule that includes community/home

Qualifications

• Bachelor's degree in Social Work or related field documented.
• Experience with case management or care coordination preferred.
• Knowledge of regional center systems and person-centered planning is a plus.

Responsibilities

• Conduct intake assessments and develop individualized service plans.
• Manage and monitor assigned caseloads.
• Coordinate services with Regional Centers, healthcare providers, schools, and community agencies.
• Assist families in accessing IHSS, SSI, Medi-Cal, CalFresh, transportation, employment, housing, respite, day programs and recreation.
• Advocate for participants and identify solutions to barriers.
• Provide education, guidance, and ongoing support to individuals and caregivers.
• Participate in IPP and interdisciplinary team meetings.
• Maintain accurate timely documentation and logs.
• Meet productivity, documentation, and compliance standards.
• Attend trainings and staff meetings.
