Job summary

Thrive Support Services Inc. seeks a Coordinated Family Support (CFS) Specialist to provide case management and supportive services for adults with intellectual and developmental disabilities choosing to stay in the family home.

You will collaborate with families, service providers, and community agencies across Contra Costa and Alameda Counties. The role involves coordinating resources, facilitating service delivery, and promoting stability and independence with a small caseload.

Qualifications

• 2-4 year degree preferred in human services or related field.
• At least 1 year of experience in developmental disabilities or related field.
• Combination of education and experience acceptable.

Responsibilities

• Identify and coordinate supports for individuals to remain in the family home.
• Conduct intake, assessment, and service planning.
• Assist with navigation of Medi-Cal, IHSS, Social Security.
• Facilitate education related to diagnosis and needs.
• Coordinate consistency and quality of services from support professionals.
• Prepare and submit service progress reports.
