Safe Harbor in Erie, PA, is seeking a compassionate Service Coordinator I to join our case management team. In a hybrid role, you will connect students and families with vital community resources across schools, the community, and clients' homes, coordinating mental health services and support.

You will work with school staff and family services, develop service plans, maintain records, and stay current with best practices.

Qualifications

• Bachelor's degree in Human Services field required.
• 12 credits in Human Services coursework required or considered.
• One-year related experience preferred.
• Prior experience in community settings preferred.
• Valid driver's license required.
• Bachelor level transcripts must be attached with application.

Responsibilities

• Coordinate mental health services and promote recovery.
• Develop and implement comprehensive service plans with individuals and families.
• Facilitate interdisciplinary meetings to ensure needs are met.
• Connect individuals with internal and external resources.
• Maintain accurate records and stay up to date on best practices.
