Job summary

The role coordinates services among community providers, schools, and healthcare professionals, ensuring timely access to supports and monitoring progress toward goals, with hybrid work and occasional in-person meetings in Yolo, CA.

Qualifications

• Demonstrated Service Coordination skills, including case management, care planning, and resource navigation.
• Strong interpersonal and communication abilities to build trust with diverse individuals, families, and partners.
• Experience in operations management or related tasks, organizing priorities and ensuring regulatory compliance.
• Customer service focus with empathy, problem-solving, and professional responsiveness to clients.
• Knowledge of or willingness to learn about the Lanterman Act, Early Start, and community resources.

Responsibilities

• Manage a caseload of individuals and families through intake, assessment, and development of person-centered service plans.
• Coordinate services among community providers, schools, healthcare professionals, and families; ensure timely access to supports.
• Maintain accurate and timely documentation; participate in interdisciplinary meetings and respond to inquiries.
• Support individuals and families in navigating resources and advocacy efforts; collaborate with team members on outreach.
