Job summary

WTC Support Services is hiring a Part-Time Support Coordinator to manage a caseload of individuals receiving Coordinated Family Support (CFS) services in Inland Empire, CA. The role combines community visits, home visits, and remote work within a flexible hybrid model.

You will conduct assessments, develop individualized plans, coordinate resources with Regional Centers, and advocate for families to overcome barriers while maintaining detailed documentation and collaborating with community

Qualifications

• Excellent communication, organization, and time management skills required.
• Strong critical thinking and problem-solving abilities.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Conduct intake assessments and develop individualized service plans.
• Manage and monitor assigned caseloads.
• Coordinate services with Regional Centers, healthcare providers, schools, and community agencies.
• Assist families in accessing resources such as IHSS, SSI, Medi-Cal, CalFresh, transportation, housing, and day programs.
• Advocate for participants and address barriers to services.
• Provide education, guidance, and ongoing support to individuals and caregivers.
• Attend IPP meetings and interdisciplinary team meetings.
• Maintain timely documentation and ensure compliance with standards.
• Meet agency productivity and reporting requirements.
• Attend required trainings and staff meetings.
