Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources. You will develop and update Individual Support Plans, ensure service quality, and advocate for participant choices.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% to meet participants.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and life goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure proper services.
• Provide participants with choice and advocacy for providers and care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources to deliver quality care.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
