Job summary

Care Lync is seeking a Support Coordinator to help adults with disabilities and aging residents access housing, medical care, community engagement, and independence. You will conduct person-centered assessments and develop Individual Support Plans to ensure participants receive appropriate services.

This role requires strong organizational and advocacy skills, independence, and willingness to travel up to 50% to meet participants in homes and communities.

Qualifications

• Bachelor's degree required.
• 3+ years of experience in case management or support coordination.
• Strong organizational, communication, and advocacy abilities.
• Willingness to travel up to 50% to meet participants in homes and communities.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Conduct person-centered assessments to identify needs and goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in care options.
• Coordinate with medical professionals, family, and community resources.
• Monitor services for safety, satisfaction, and progress toward independence.
• Maintain documentation and ensure confidentiality and compliance.
• Participate in ongoing training and professional development.
