Job summary

Care Lync is seeking a dedicated Support Coordinator to develop individualized care plans and coordinate services for adults with IDD, Autism, and physical disabilities. You will broker services, monitor outcomes, and ensure cost-effective care aligned with ISP goals.

The role involves location-based service coordination, housing, medical and community supports, plus regular collaboration with supervisors and other coordinators. Travel up to half the time is required.

Qualifications

• BS/BA degree in related field required.
• Minimum of 3 years case management experience with IDD/Autism/Physical Disabilities.
• Proficiency in Microsoft Office and state databases.
• Maintain required trainings and CPR/First Aid certification.
• Valid driver's license and vehicle insurance on file.

Responsibilities

• Complete assessments to identify participant needs and desires for services.
• Develop Person-Centered Service Plans and update annually or with changes.
• Ensure participant provider choices and ISP alignment.
• Coordinate with care teams to ensure integrated services.
• Maintain confidentiality and participate in required trainings.
• Travel to participants up to 50% of the time.
