Job summary

Department of Economic Security (DES) in Globe, AZ seeks a Case Manager 2 to support individuals with developmental disabilities and their families. Join a team focused on independence, community engagement, and service coordination.

Prior experience in case management and understanding of PCSPs are essential. The role requires a valid driver’s license, ability to drive, and a commitment to confidentiality.

Qualifications

• Knowledge of HIPAA and confidentiality requirements.
• Ability to develop and evaluate Person-Centered Service Plans (PCSP).
• Experience with case management for individuals with developmental disabilities.

Responsibilities

• Drive to members' homes, schools or sites to discuss service needs.
• Develop and monitor PCSP ensuring timeliness and referrals.
• Facilitate meetings and complete referrals for services.
• Collaborate with community agencies to assist members and families in crisis.
• Prepare progress notes and various reports.
