Job summary

Corewell Health Southfield Center invites applications for a full-time role in the Behavioral Program Blue Envelope Crisis Support - Medical Group West. The position involves coordinating services across multiple disciplines within and outside the health system, with a focus on education for patients and families and outcome measurements.

Day shift, 40 hours weekly, Monday–Friday, 8:00 a.m.–4:00 p.m. Minimum requirements include a Bachelor’s degree; MI RN/LPN or équivalent upon hire, or

Qualifications

• Bachelor's degree or equivalent in a related field.
• Master's degree or equivalent in a related field preferred.
• 2 years of relevant related experience required.
• MI-licensed RN required upon hire.
• MI-licensed LPN required upon hire.
• RD credential required upon hire.
• PA-C credential required upon hire.
• MSW preferred; relevant mental health/behavioral health experience helpful.
• Licensed Physical Therapist required upon hire.
• Licensed/Registered Occupational Therapist required upon hire.
• Speech-Language Pathologist credential required upon hire.
• Licensed Respiratory Therapist required upon hire.
• Athletic Trainer certification/licensure required upon hire.
• Athletic Trainer License (MI) required upon hire.
• Nuclear Medicine Technologist certification required upon hire.
• Experience working as a Master Social Worker strongly preferred.

About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. Scope of work

Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
