Job summary

Corewell Health Southfield Center in Michigan is seeking a licensed professional to coordinate multidisciplinary services in outpatient settings. The role involves assessment, protocol initiation, patient education, and program outcome evaluation within the Behavioral Program Blue Envelope Crisis Support team.

Required are a related field degree and MI licensure at hire; 2 years of experience strongly preferred, with day shifts and 40-hour weeks in a hospital network.

Qualifications

• Bachelor's degree or equivalent in a related field is required.
• Master's degree or equivalent is preferred.
• Minimum 2 years of related experience required.
• Licensure required at hire for MI-listed roles (RN/LPN/PA-C/RD/OT etc.).

Responsibilities

• Coordinate services across multiple disciplines inside and outside the health system.
• Assess and initiate protocols/procedures with independent judgment.
• Provide educational support for patients, family, health team and community.

About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Scope of work

Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
