Job summary

Community Health Resources in Windsor, CT is seeking a part-time Care Coordinator to support the Zero Suicide program. You will participate as a member of a multidisciplinary treatment team, coordinate client intake and services, and assist with treatment planning and daily triage in a collaborative setting.

The role requires a Bachelor’s degree in a human service-related field and at least two years of related experience, plus a valid driver’s license.

Qualifications

• Bachelor’s degree in a human service-related field required.
• A minimum of two years’ related experience preferred.
• Valid driver’s license required.
• CPR/First-Aid training may be required within 6 months of hire.

Responsibilities

• Participate as member of multi-disciplinary treatment team.
• Track and coordinate client intake, discharge, services and referrals.
• Assist with treatment planning and daily triage.
• Educate and link individuals to self-help groups and community resources.
• Assist with linking clients to benefits such as insurance, housing, and medical services.
• Maintain documentation to ensure quality services and compliance.
• Coordinate care between the agency and other providers.
• Assist with scheduling and transportation to appointments.
• Maintain program standards in documentation.
• Attend staff meetings, supervision, training, and other sessions.
• Understand cultures and communication styles to provide culturally sensitive care.
• Provide crisis intervention and support as needed.
