Mental Health Resource Center is seeking a Care Coordination – Suicide Prevention Team Leader to coordinate Zero Suicide services across crisis units, inpatient facilities, and EDs. You will ensure timely transitions, oversee risk assessments, and guide care plans with evidence-based interventions.

You will supervise Care Coordinators and Clinicians, train partners, monitor data outcomes, and lead quality assurance while participating in a 24/7 on-call rotation to support high-risk populations.

Qualifications

• Bachelor’s degree from an accredited university or college in counseling, social work, psychology, criminal justice or related Human Services field with three years’ experience working with adults experiencing serious mental illness; supervisory experience required or Master’s degree with one year supervisory experience.
• Proficiency in Microsoft Office, Outlook and Internet usage; RBHS/MHRC EHR and Patient Information System proficiency within three months of employment.
• One year of supervisory experience required (for Master’s degree path) or three years’ experience (for Bachelor’s path).
