Job summary

Mental Health Resource Center, Inc. is seeking a full-time Care Coordination - Suicide Prevention Team Leader to coordinate Zero Suicide services across crisis units, inpatient facilities, and emergency departments.

You will ensure protocol compliance, lead interagency collaboration, and oversee risk assessments and safety plans for high-risk adults. The role requires supervisory experience, advanced degrees in counseling/psychology or related fields, and proficiency with EHR systems.

Qualifications

• Bachelor's degree in counseling, social work, psychology, criminal justice or related field; 3 years' experience with adults with serious mental illness; 1 year supervisory.
• Master's degree in related Human Services field; 1 year supervisory experience required.
• Proficiency in Microsoft Office, Outlook and the use of the Internet; EHR system proficiency required within 3 months of employment.

Responsibilities

• Outreach and Interagency Collaboration: Lead in providing outreach to crisis units, inpatient facilities, emergency departments, shelters to promote Link to Life and strengthen partnerships.
• Protocol and Workflow Development: Establish and refine protocols with partners to align with Zero Suicide and ensure seamless care transitions.
• Service Delivery and Compliance Oversight: Monitor screenings, risk assessments, and follow-up to ensure adherence to Zero Suicide and MHRC standards.
• Assessment Review: Review risk assessments and safety plans; ensure they are comprehensive and reflect multidisciplinary input.
• Risk Flag Management: Oversee risk flags to enroll moderate/high risk individuals in the Pathway within 24 hours.
• Safety Planning and Lethal Means Counseling: Develop individualized safety plans and provide lethal means counseling; document updates the same day.
• Follow-Up Protocols: Ensure structured follow-ups at 24-48 hours, weekly first 30 days, then up to 180 days.
• Transition and Discharge Planning: Guide transitions to lower levels of care with coordination among providers.
• Team Supervision: Supervise Care Coordinators and Clinicians; conduct regular meetings to review caseloads.
• Productivity and Quality Assurance: Monitor productivity and data accuracy; ensure policy adherence.
• Direct Intervention: Provide direct service as needed and back-up for crises.
• Clinical Guidance and Case Oversight: Support complex cases with expertise; ensure high-quality care.
• On-Call Rotation: Participate in 24/7 on-call rotation for emergencies.
• Reporting and Documentation: Compile monthly reports; ensure accurate documentation and billing.
• Internal and External Training: Develop and deliver training on Zero Suicide and Link to Life.
